Cardiogram is a “primary care of the future” digital health provider built on consumer wearables available today, such as Apple Watch, Wear OS by Google, Fitbit, Garmin and Fossil. We monitor individuals for signs of undiagnosed diabetes, hypertension, sleep apnea, and atrial fibrillation, confirm using standard diagnostic tests, and then guide patients to the next step of treatment. Cardiogram also supports ongoing remote monitoring of chronic conditions for secondary prevention purposes including an innovative Share with Family and Share with Doctor capability. Over 80% of people with sleep apnea are undiagnosed. For people with diabetes, hypertension, and atrial fibrillation, anywhere between one-fifth and one-third are undiagnosed. These are people with untreated, worsening chronic conditions that are at a significantly higher risk of developing costly medical complications, like strokes and heart failure. Cardiogram uses wearable devices to close this diagnosis gap, preventing medical emergencies, reducing long-term medical spend, and improving wellbeing.
