Hank Schlissberg is the president of Vively Health, formerly DaVita Health Solutions and now a standalone subsidiary of DaVita. Vively provides in-home primary care for a health plan’s highest risk, chronically ill members, who they call Most Vulnerable Patients (MVPs).As a full risk provider group, Vively Health goes far beyond DaVita’s core kidney care focus to enhance the care model for MVPs through its comprehensive house calls program—which includes primary care, social and behavioral health, palliative care, medication management, post-acute care, 24/7 clinical access and other services.Prior to joining DaVita in July 2014, Hank was a Partner at McKinsey & Company and leader in the payor-provider practice, where he joined the Cleveland office in 2000 and remained into 2011. He then spent two and a half years as Chief Growth Officer of Radisphere, a $40M venture-backed (Oak HC/FT, Maverick Capital) technology and delivery model innovator in the radiology space, before it was sold to Envision Healthcare in 2014. Hank has an MBA from the University of Michigan and played varsity golf at Miami University.Hank lives in Denver with his wife Carlynn and their three young girls where they love the Colorado outdoor lifestyle and family ski weekends. He still tries to be a somewhat competitive golfer and is a passionate Michigan State and Cleveland sports fan.
